Azara Healthcare, the four-time Best in KLAS provider of population health and value-based care solutions, today announced that Mary Lantin joined the company as President and Chief Executive Officer, effective June 15, 2026. Jeff Brandes, Azara’s founder and longtime President & CEO, assumed a full-time leadership role as Founder, Executive Advisor, and member of the Board of Directors. The planned transition reflects Azara’s strong momentum and positions the company to continue expanding its impact for providers and health plans serving underserved communities.
Azara supports more than 1,000 healthcare organizations across all 50 states, helping improve care and outcomes for more than 50 million patients nationwide. The company serves federally qualified health centers and other safety-net providers, community and critical access hospitals, and health plans with data-driven solutions that improve quality, strengthen value-based care performance, and support regulatory reporting.
Since teaming with Insight Partners in late 2024, Azara has advanced its vision to define the next generation of payer-provider collaboration, creating a unified foundation for trusted data, shared workflows and measurable outcomes that help turn partnership intent into operational results and advance the impact of value-based care.
“Azara has always been built around mission, innovation and a deep commitment to the organizations and communities we serve,” said Jeff Brandes, Founder of Azara Healthcare. “This is a planned transition initiated as part of our long-term leadership planning, and it reflects both the strength of the business and the opportunities ahead. While I am transitioning from day-to-day leadership, I will remain actively engaged with Azara, working closely with clients, supporting strategic initiatives and contributing as a member of the Board. Mary is a values-driven, strategic and client-centered leader, and I am excited to partner with her in this next chapter.”
Lantin brings more than two decades of leadership experience in healthcare technology, with deep expertise in healthcare data, analytics, interoperability, operations strategy, and enterprise growth. She most recently served as Senior Vice President, Operations Strategy & Partnerships at Centene. Prior to that, she held senior leadership roles at Availity, Diameter Health, Optum Analytics, and Humedica, helping organizations scale, integrate teams and solutions, and deliver measurable value across payer and provider markets. Across her career, Mary has been committed to unlocking the value of data to generate actionable insights, improve decision-making, enhance operational efficiency, and advance better healthcare outcomes.
“I am thrilled to join Azara Healthcare at such an important time in the company’s evolution,” said Mary Lantin. “Azara has built an exceptional reputation for helping providers and health plans turn complex data into meaningful action, especially in the safety net, where the opportunity to make a difference is significant. What drew me to Azara is not only the strength of its platform and market position, but also its mission, culture and commitment to clients. I am excited to work with Jeff, the leadership team, and employees across the company to build on that foundation and help accelerate Azara’s next phase of growth.”
“Azara is exceptionally well-positioned for its next stage of growth,” said Krishnan Sastry, Managing Director at Insight Partners and member of Azara’s Board of Directors. “Healthcare organizations increasingly need trusted partners that can connect data, improve performance and strengthen collaboration across providers and health plans. We believe Azara is uniquely positioned to meet that need, and Mary's leadership will help accelerate the company's ability to deliver value at scale.”
Under Brandes’ leadership, Azara has grown from a startup into a leading provider of population health and value-based care solutions. With a growing presence across providers and health plans, Azara remains focused on helping healthcare organizations improve outcomes, advance value-based care and strengthen collaboration through data-driven insights.
