Patients in rural settings are generally seen by independent organizations operating on different systems and limited visibility into care beyond their own walls. Without shared data and interoperability tools, patients and their clinicians struggle to navigate next steps, and care coordination becomes fragmented as patients move between facilities.

Patients are also able to access a single, unified view of their entire medical history through a smartphone or home computer.
