Modern Healthcare Names Aledade CEO Farzad Mostashari Among 50 Most Influential Clinical Executives of 2026

Modern Healthcare has recognized Farzad Mostashari, MD, co-founder and CEO of Aledade, a physician‑led, national leader in value‑based care, as one of the 50 Most Influential Clinical Executives of 2026. According to Modern Healthcare, the 50 Most Influential Clinical Executives use their medical expertise to overhaul care models, launch workforce development programs and drive revenue growth at their organizations.

Modern Healthcare selected Mostashari, a board-certified internal medicine physician, for his leadership in expanding value-based care and strengthening primary care organizations so they can succeed and grow. For the 2026 performance year, there are 511 accountable care organizations (ACOs) participating in the Medicare Shared Savings Program (MSSP), up from 476 in 2025. Aledade currently partners with more than 3,000 primary care organizations across 46 states, including more than 370 community health centers. Together, these partners participate in Aledade ACOs that support more than 3 million patients.1

Aledade enables primary care organizations to deliver high-quality proactive, preventive care. In 2024, Aledade partners helped prevent 263,000 unnecessary hospitalizations and emergency department visits, conducted more than 800,000 annual wellness visits2 and achieved an 83.3% patient hypertension control score3 — 4% higher than the national average.2 In the most recent MSSP performance year of 2024, 93% of Aledade’s ACOs achieved shared savings, generating more than $1 billion in Medicare savings.4

Before launching Aledade, Mostashari served as the National Coordinator for Health Information Technology, leading federal efforts to modernize electronic health records nationwide. His experience shapes Aledade’s physician-led approach, which delivers actionable insights directly into clinical workflows through technology solutions, including the Aledade AssistTM electronic health record overlay, and coaching.

Mostashari, who brings over 25 years of clinical and health policy expertise to Aledade, earned a Bachelor of Arts in Biochemistry from Harvard University, a Master of Science in Population Sciences from the Harvard School of Public Health and a Doctor of Medicine from the Yale School of Medicine. He co-founded Aledade in 2014 out of a critical need to fix the misaligned incentives of the traditional fee-for-service system.
