While heart disease remains a leading cause of death in the United States – with nearly half of all American adults experiencing high blood pressure and only one in four of those affected having it under control – Aledade partners are leveraging data and innovation to radically change these outcomes.
In 2024, primary care organizations participating in the Medicare Shared Savings Program (MSSP) that have partnered with Aledade for two years or longer achieved an 83.3% average for hypertension control.1 This success rate outperforms some of the largest health systems in the nation and sits 4% higher than the national average.
By prioritizing high-quality, preventive care, Aledade partners not only save patients from life-threatening heart attacks and strokes, but they also contribute to a more sustainable health care system. Aledade’s MSSP partners saved over $1 billion for the Medicare Trust Fund while avoiding nearly 263,000 unnecessary hospitalizations and emergency visits in 2024 alone.
